Keeping a symptom diary is a great way to document this information. We have prepared forms to make keeping a symptom diary as simple as checking a few boxes and making brief notes each day. By providing a detailed picture of how your disabilities affect you, you can substantially improve your chances of winning your Social Security disability claim. Our forms are available for download below. If you are a current client, print them and send them to us at the end of each month. We will submit them to SSA as part of your disability claim.
